Adderall and Elvanse both treat ADHD using amphetamine-based mechanisms, and Elvanse is generally considered the closest UK-licensed equivalent to Adderall — but they aren't the same medicine.

What each one is

Adderall is a US brand of mixed amphetamine salts — a fixed combination of dextroamphetamine and levoamphetamine (roughly a 3:1 ratio in most formulations). It has no UK marketing authorisation.

Elvanse is the UK brand name for lisdexamfetamine, a prodrug. It's pharmacologically inactive when taken and is converted by the body — mainly through enzymes in red blood cells — into dexamfetamine, the active stimulant. Dexamfetamine is one of the two amphetamine isomers present in Adderall.

How the mechanism differs

This prodrug conversion is the key practical difference. Because lisdexamfetamine has to be metabolised before it becomes active, its onset and offset tend to be smoother and more gradual than immediate-release amphetamine salts, and its release can't be sped up by routes of misuse that bypass normal absorption (such as crushing and snorting), since the conversion step still has to happen in the body regardless of how it's taken. This is one reason lisdexamfetamine is generally considered to have a somewhat different misuse-risk profile compared with non-prodrug stimulants, though it remains a Schedule 2 controlled drug requiring the same prescribing safeguards.

Dosing and duration

Elvanse is taken once daily and is designed to provide coverage across the day from a single dose. Adderall is available in both immediate-release (multiple daily doses) and extended-release (Adderall XR, once daily) formulations in the US. Because the two medicines use different active mechanisms, doses are not interchangeable on a milligram-for-milligram basis — this is always a clinical decision made by a prescriber, never something to work out independently.

Which is "better"?

Neither is universally better — effectiveness and tolerability are individual. Someone who previously used Adderall abroad and is now looking at UK options should discuss their full history with a UK specialist, who can make an informed recommendation based on their specific response history, rather than assuming a direct swap.
